Registered Dietitian - Primary Care

Registered Dietitian  needed for a  part-time, temp contract for about 2 months or potentially temp-to-hire  opportunity with Yoh’s well-known client located in Chandler, AZ.

This clinic offers occupational and primary care services for employees of a large corporation, including their families, ages 5+.

--> SHIFTS: 16 hrs/week - Tuesday/Wednesday 7:00am to 3:30pm  and if this goes temp-to-hire, will turn into 20 hrs/week - Tues 7:00am to 3:00pm (remote), Wed 9:00am to 5:00pm (onsite), Thurs 7:00am-12:00pm (remote) 


Top Skills You Should Possess:

• Bachelor's Degree with course work approved by the Academy of Nutrition and Dietetics
• Minimum (3) years' experience as a Registered Dietitian in a clinical setting
• Experience with public speaking and presentations (may provide nutrition wellness programming and presentations to engage the client population)
• Prefer someone experienced in health promotion or worksite wellness

What You’ll Be Doing:

• Provide individual nutritional assessments, evidenced based education and support to members to help them develop and maintain healthier eating habits
• May prescribe Medical Nutrition Therapy to help improve health conditions and clinical outcomes
• Conduct a comprehensive nutritional assessment on members ages 2 and up, which includes obtaining a detailed nutrition history, review of risk factors, labs, and medical history, and identification of nutrition behaviors contributing to risk
• Identify and discuss areas where members can decrease health risks and provide evidence based recommendations, education, and resources or provide referrals as needed
• Develop an individualized plan-of-care and assist members in creating SMART goals while evaluating progress and following up as needed
• Document nutrition care plans and member progress 
• Review HEDIS standards for members who have Diabetes or HTN and monitor outcomes
• Plan and present nutrition wellness programs
• May work with cafeteria or food service vendor to evaluate nutrition offerings and make suggestions

What You Need to Bring to the Table:

• Copy of Commission on Dietetic Registration Certificate Card and CDR number (National Commission on Dietetic Registration)
• Computer skills with previous experience using EMR system, EPIC
• Yearly TB Test or QuantiFERON Gold
• Interpersonal skills necessary to interview, teach and counsel patients
