Intake Coordinator

Intake Coordinator needed for a contract opportunity with Yoh’s client located in San Francisco, CA! **WORK FROM HOME**

Top Skills Should You Possess:

• Minimum one (1) year of experience in Triage, Intake or Care Coordination

What You’ll Be Doing:

• Ensure complex populations get connected to the most appropriate services available
• Provide the primary intake and triage services for telephonic referrals and inquiries to all care coordination programs within the organization and the external provider network
• Manage incoming calls to the Care Management Intake Line
• Receive and process inquiries for all Care Management programs from internal referrals, community providers, emergency departments, primary care providers and other agencies
• Provide outstanding customer services to internal and external customers
• Triage and process referrals into the appropriate CM programs, actively seek consultation from clinical supervisors when needed to escalate a referral and support the Notice of Action (NOA) process
• Effectively manage the Community Supports services referral process including referrals from SF Medical Respite
• Triage and de-escalate calls from members who may be in crisis
• Schedule: Monday – Friday, 8am – 5pm or 8:30am – 5:30pm (Pacific)

What You Need to Bring to the Table:

• Associates or Bachelors Degree in related field; or 4 years of relevant experience
• Minimum one (1) year of experience in Triage, Intake or Care Coordination
• Bi-cultural, bi-lingual language skills preferred
• Experience working with people with complex health needs
• Working knowledge and use of Trauma Informed language and principles a plus
• Working knowledge and use of Harm Reduction language and principles a plus
• Working knowledge and use of Person-Centered language and principles a plus
• Experience with de-escalation techniques
• Telephonic Case Management experience a plus
• Excellent verbal and written communication skills
• Excellent computer skills
